In order to reset your password, you must first complete the information below. Please enter your Login ID (First Initial + Full Last Name + Middle Two Digits of your SSN), date of birth and the last four digits of your Social Security Number.

Once you have entered this information correctly and selected "Reset Password" your password will be reset to your Login ID (First Initial + Full Last Name + Middle Two Digits of your SSN). You will then be asked to change your password upon entering the Online Benefits Center.
